whats the medical reason?

I think it can be a combo of the body not being used to nicotine and maybe more carbon monoxide than is good. Nicotine does not bother me much, but I have seen people get sick and puke after smoking a cigar. Nicotine seems to hit some harder than others. People who have never smoked before could be in for a surprize.

Too much carbon monoxide causes bradycardia (slow heart rate) and usually the nasty headache, drowsiness and nausea and vomiting.
Nicotine causes the heartrate to increase, and also the headaches, nausea and vomiting etc.
So when you get a mixture of the both...good things aren't bound to happen. Nicotine is also like anything else out there, people can have sensitivies to it, just like some people drink a cup of coffee and are fine, while others get all twitchy and hyper.
Just enjoy in moderation and you'll have no problems...
